Headphone jack stopped working

I recently replaced my headphone jack, and the new one stopped working after a couple of months.

It is not detected in windows, and I tried to unplug/reseat the cable from the audio board to the motherboard.

I cannot see the audio device in windows.

Is this possibly a hardware issue or a device driver issue? Any ideas?

Thanks JB. In this case, the issue went away after updating all drivers, BIOS, and then reinstalling the audio board itself. It seems like the connection from MB to Audio board might have been the issue in this case.
